EDOF IOLs: Extended Depth of Focus Lenses for Cataract Surgery
By Dr Ross MacIntyre
Extended Depth of Focus (EDOF) intraocular lenses represent one of the most significant advances in cataract surgery over the past decade. For many patients, they offer an excellent middle ground between a standard monofocal lens and a full multifocal IOL.
What Makes EDOF Lenses Different?
Traditional monofocal lenses focus at a single point — typically distance. Multifocal lenses create multiple distinct focal points simultaneously. EDOF lenses take a different approach: they elongate the focal range into a continuous zone, providing clear vision from distance through to intermediate (computer distance) without the discrete "jumps" between focal points that multifocal lenses create.
The result is vision that feels more natural, with fewer halos and less glare than multifocal options — while still significantly reducing dependence on glasses for most daily tasks.
What Can I Expect to See Clearly?
With an EDOF lens, most patients achieve spectacle-free vision for:
- Driving and outdoor activities
- Computer and tablet use (50–70 cm)
- Watching television
- Most intermediate tasks
Some patients still need glasses for reading very fine print, particularly in low-light conditions. However, for many people, this is an acceptable tradeoff compared to the stronger optical side effects of multifocal lenses.
EDOF vs Multifocal IOLs
| | EDOF | Multifocal | |---|---|---| | Vision range | Distance to intermediate | Distance, intermediate and near | | Halos / glare | Lower rates | Higher rates | | Contrast sensitivity | Excellent | Varies | | Reading glasses needed | Sometimes (fine print) | Rarely | | Best for | Drivers, computer users | Full glasses independence |
Who Is a Good Candidate for an EDOF Lens?
EDOF lenses are well suited to patients who:
- Prioritise distance and intermediate vision (driving, computer)
- Are sensitive to visual disturbances such as glare or halos at night
- Have mild ocular conditions that make them suboptimal candidates for multifocal lenses
- Accept that reading glasses may occasionally be needed for small print
Available EDOF Platforms
Several well-established EDOF lens platforms are available in Australia, including the Tecnis Symfony, Alcon Vivity, and HOYA Vivinex iSert XY1C. Each has slightly different optical characteristics, and the choice of platform is tailored to the individual patient's measurements and requirements.
Finding Out If EDOF Is Right for You
During your cataract surgery consultation, I will perform detailed measurements of your eye, assess your corneal health, and discuss your visual priorities in detail. For many Melbourne patients, EDOF lenses offer an excellent blend of optical quality and reduced glasses dependence.
